2.4.13 Transfer to another facility

1 Purpose and objective

Establish a procedure for selecting a suitable treatment centre according to the treatment needs (specialised treatment, sub-acute care, rehabilitation centre. The criteria for the selection must be set out, the notification of the receiving institution, the transport conditions, formalities of the transfer (possibly general agreements for the transfer). Agreement on the time of transfer, responsibility for transport, provision of medicines or medical devices, monitoring of the process until arrival at the receiving facility.
Procedure if relocation is not possible.
