Goal and purpose

Discharge from treatment should be planned as a separate, closed process. Consideration should be given to how further treatment and care will be ensured (continuity of treatment, see also section 2.5 "Aftercare")
Discharge planning should begin at the earliest possible point in time.
The criteria for discharge should be defined, in particular patient safety criteria.

Discharge must be coordinated with those who will provide medical and other care.
