2.5.08 Transfer to another hospital

Aim and purpose

Indication, transportation, necessary information and agreement on readmission Selection of a suitable treatment facility according to the treatment needs (specialist treatment, sub-acute care, rehabilitation facility).

The criteria for the selection must be set out, the notification of the receiving institution, the transportation conditions, formalities of the takeover (possibly general agreements for the takeover).

• Agreement on the time of transfer, responsibility for transportation, provision of medicines or medical devices, monitoring of the process until arrival at the receiving facility.
• Procedure if relocation is not possible.
• Accompanying documentation during relocation
