This procedural instruction does not cover hazardous work for which the managing director, as the client, has delegated responsibility in writing to a third party. A third party in this sense can be a natural person (e.g. architect, engineer or building contractor) or a legal entity (e.g. a planning office as a limited company). Furthermore, hazardous work in accordance with Annex II of the Construction Site Ordinance, for which a health and safety plan is drawn up, is not covered by these procedural instructions. 2.1 Work with increased risk At Sankt- Katharinen- Hospital GmbH, such work can be, for example - Welding, cutting, soldering, thawing and separating work - Work on gas pipes (e.g. natural gas, oxygen) - Working on pressurised devices and pipes (e.g. steam pipes, pressurised gas cylinders) - Work on devices/components that are live (e.g. switchgear) - Work involving a risk of falling, e.g. on roofs and other high workplaces, e.g. building facades, windows - Work carried out by persons outside the area in areas where hazards due to ionising radiation may occur, e.g. cleaning staff - Work carried out by external personnel where there is a risk of exposure to highly toxic, carcinogenic or mutagenic substances, e.g. when working on sanitary facilities in areas where patients are treated with cytostatics - Work by external staff in areas where there is a risk of infection - Working with hazardous equipment 2.2 Devices for which accident prevention regulations prescribe written authorisation for operation - Motorised industrial trucks (forklift trucks, pallet trucks) - Motor vehicles of the hospital - Winches, lifting and pulling devices - Load handling attachments in hoist operation - Lifting platforms 3 Description Safety measures and supervision when carrying out hazardous work are specified in a permit (individual permit) or, in the case of frequently occurring hazardous work, in a service instruction/operating instruction. The safety measures for operating appliances with increased hazards are specified in the operating instructions for such appliances. Knowledge of these operating instructions is a prerequisite for written authorisation of the employee to operate the appliance by the managing director. 3.1 Permit (work permit) 3.1.1 Contents of the licence For welding, cutting, soldering, thawing and cutting processes, a permit is introduced in the hospital (Appendix 2). For the other work mentioned under point 2 with increased risk, a permit is issued if necessary in accordance with the model in Appendix 1: - Work location/position - Work order e.g. welding on bracket - Type of work e.g. welding, cutting etc. - Safety precautions before starting work e.g. removing all flammable objects and materials - Safety precautions during work e.g. fire watch - Safety precautions after work e.g. fire watch - Alarm routes e.g. location of the nearest fire alarm and telephone as well as the telephone number of the fire brigade - Extinguishing equipment, e.g. fire extinguisher with CO2 - Authorisation to carry out hazardous work on condition that the listed safety measures have been implemented. - the validity date - Signature of the managing director or his authorised representative (usually head of technology) - Signature of the performer 3.1.2 Handling the licence The authorisation form is issued and signed by the Head of Technology or his deputy before the start of work. The above-mentioned items are filled in for the specific job and order and the safety measures before, during and after the work are specified. - The Head of Technology or a person authorised by him explains the contents of the permit and the possible hazards and safety measures to the person responsible for the work (including an external company) before work begins. - The person responsible for the execution signs the authorisation form - The person responsible for execution shall be instructed at the work site by the client or a person authorised by the client. 3.1.3 Validity of the licence The licence is only valid if it is completed in full and bears both signatures. The permit is valid for the specified period. The technical manager keeps a copy of the licence, the original is kept by the person carrying out the work. 3.1.4 Return of the licence The permit must be handed in to the Head of Technology or his deputy immediately after completion of the work. 3.2 Service and operating instructions For frequently recurring hazardous work, the safety measures in the hospital are defined in service or operating instructions, e.g. in the service instruction "Radiation protection" (3.2.06) and in the two operating instructions in accordance with the Hazardous Substances Ordinance "Cytostatics" (production) and "Cytostatics" (application) (BA 8.3.01 and 8.3.02). Proof of annual instruction in the safe handling of these substances is required. The safety measures for work in hazardous areas carried out by the cleaning company are set out in procedural instruction 3.2.02 "Safety instruction in the cleaning service". 3.3 Written assignments The hospital's managing director authorises the driving of motorised industrial trucks and motor vehicles in writing. The authorised persons are recorded in a list kept by the Head of Technology.
